At Adaptive, our RN Case Managers focus on performing in-home admissions, in-home supervisory visits, office duties, and semi-annual or annual on-site assessments of field caregivers. Our teams are dedicated to ensuring compliance with the client assessment of need and service plan standards that are compliant with state regulations.

Office hours/work hours for this role are normally Monday - Friday 9:00AM - 5:00PM, but will vary to support business needs.

A typical day for a RN Case Manager includes:

  • Conducting initial evaluation visits and regularly re-evaluating clients' nursing needs.

  • Counseling clients and their families regarding nursing needs and related needs.

  • Coordinating and creating service plans for clients following client assessments and admissions.

  • Assisting and supervising field staff, ensuring successful implementation of new Medicaid Waiver programs.

  • Ensuring quality care is delivered to our clients by creating, implementing, monitoring, and enforcing quality assurance policies and procedures.

  • Assisting our branch teams with pre-service training and orientation, as well as in-service training for caregivers.

  • Assisting with other duties as needed and as assigned by the Branch Manager.

In order to be eligible for this role, you should have:

  • A current RN license in the state of services

  • A valid driver's license

  • Access to reliable and insured transportation

  • Ability to travel within the surrounding counties 2-3 days per week between the hours of 9:00am - 5:00pm

  • Nursing experience preferred

RN Case Manager will be eligible to receive a $2500 hiring bonus after 60 days of employment
