Job Summary:

The Supervisory LPN is responsible for monitoring the personal care and companion services

provided by company’s direct care staff; coordinating and delivering clinical services, ensuring quality

of care and assisting with building referral relationships.

Supervisory Responsibilities:

• None.

Duties/Responsibilities:

• Perform in home assessments and evaluations to determine support and client needs.

• Conduct regularly scheduled supervisory visits of clients.

• Monitor personal care and companion care services provided by direct care staff

• Assist in educating direct care staff to ensure quality of care.

• Assist in building referrals and referral sources.

• Perform other related duties as assigned.

Required Skills/Abilities:

• Satisfactory communication skills, oral and written.

• Emotional and mental maturity for the establishment and maintenance of good working

relationships with client, client’s family, caregiver, administrative staff and referral

sources.

• Adequate computer skills to be able to document electronically on a tablet.

• Ability to work independently and as part of a broader care team as the situation requires.

Education and Experience:

• Current State of Missouri LPN license.

• Current CPR certification.

• Minimum 1-year LPN experience and ability to function independently in a home setting.

• Free from health problems that may harm the well-being of the client or employee.

• Must have reliable transportation and willingness to drive to outlying areas.

Physical Requirements:

• Individual must have the ability to stand and work or have mobility through mechanical

assistance.

• Must be able to lift and carry items weighing up to 20 pounds.

• Primarily sedentary which requires stooping, bending, reaching, hearing, and vision in order

to operate phone, computer, and facsimile.

• Frequent traveling to clients’ homes and referral sources using a personal vehicle.

• This position is required to work in client homes with a variety of climate conditions.
